Context
Revelation is creepy.
Okay, just had to get that out of our system. Here, John's describing a vision he sees of the Four Horsemen of the Apocalypse.
Let's see, there's the one with a bow and a crown riding a white horse (he's conquering stuff); then there's the one with a great sword who's riding a red horse (he's taking peace from the earth and making people kill each other).
So far so good.
Then there's the one riding a black horse and carrying some balances (scales) in his hand (probably to judge the world); and lastly… Death. Oh, and Hell's following him.
Anyway, those are the famous Four Horsemen of the Apocalypse. They're here to destroy the world in preparation for Christ's second coming.

Where you've heard it
Anytime there are four of something and they want to sound really hard-core, chances are pretty good that they'll call themselves "The Four Horsemen," maybe with "of the Apocalypse" thrown in for good measure.
